iHomecare Solutions, LLC is Hiring a Program Specialist Near Pittsburgh, PA

We are seeking a dedicated and detail-oriented Program Specialist to join our team. The Program Specialist will be responsible for coordinating and implementing various programs and initiatives within the organization. The ideal candidate will have strong organizational skills, excellent communication abilities, and a passion for making a positive impact.

Requirements:

The Program Specialist is required to have one of the following:

  1. A master’s degree or above from an accredited college or university and 1 year of work experience working directly with individual with intellectual disabilities or autism.
  2. A bachelor’s degree or above from an accredited college or university and 2 years of work experience working directly with individual with intellectual disabilities or autism.
  3. An associate degree or above from an accredited college or university and 4 years of work experience working directly with individual with intellectual disabilities or autism.
  4. Working knowledge applicable and regulations and requirements
  5. Current criminal history clearance

  5. Critical thinking and independent judgement and decision-making skills

Physical Demands/Requirements

  1. Ability to see and interact with text and graphical information on a screen
  2. Ability to receive and respond to phone calls
  3. Ability to use office equipment
  4. Ability to lift 50 pounds

Description of Work Environment:

  1. Regular travel to the homes of the individuals
  2. Regularly work non-traditional hours including participation in on-call throughout the week
  3. Occasionally fill in for a direct support professional shift including non-traditional hours as needed
  4. Work in an office environment or another approved location in the community

The Program Specialist is responsible for the case management of the individuals that reside at iHomecare Solutions as well as serving as an immediate supervisor to all direct support professional staff. A program specialist shall be assigned for up to 30 individuals.

Responsibilities include but are not limited to:

  1. Supervising all direct support professionals that work in the homes
    1. Include training staff and issuing disciplines if necessary
  2. Direct and participate in the development of the Individual Support Plan (ISP) that meets the needs of the individual
    1. This includes but is not limited to ISP, team meetings, text messages, phone calls, emails, and other meetings
  3. Coordinate and the timely completion of assessments and other documents required for the ISP creations, review, and revision
  4. Complete quarterly reports based on review of staff documentation and the progress of the individuals
  5. Prepare and send documentation as required
  6. Write progressive, measurable goals and monitor progress toward accomplishing goals
  7. Effective communication with family, county and staff representatives, staff, individuals, and office management
  8. Communicate changes/updates to the ISP, health, and/or behavior to the appropriate team members and staff
  9. Coordinate with the House Leads to support human resources activities to provide appropriate and trained staff in each home
  10. Assist with onboarding activities for new hires as requested
  11. Provide support and modeling to direct support professionals on ISP and encourage to promote individual choice
  12. Oversee residential services to ensure the health, safety, security, and supporting the integration of community participation of the individuals. Provide and supervise activities in accordance with the plan of the individual
  13. Support communication of the individual and involvement with family and friends
  14. Provide training and ensure proper functioning on adaptive equipment
  15. Update individual’s informational forms as needed
  16. Monitor required assessments and appointments to assure they are completed in compliance with the regulations
  17. Update individual’s records for timeliness, accuracy, and completeness in compliance with the applicable regulations
  18. Communicate in a discreet, professional, and tactful manner
  19. Complete tasks in a self-directed, organized, and efficient manner
  20. Maintain positive working relationships with staff, coworkers, county and state representatives, and family members of the individuals
  21. Assist with all QA&I, licensing, unannounced visits, and any other regulatory audits
  22. Responsible for the upkeep of the physical sites
  23. Responsible for ensuring all adaptive equipment is present and in good repair
  24. Ensures all follow up to medical appointments is completed in a timely, compliant manner
  25. Responsible to maintain all individual related charts
